Make a referral

MyCare Connect is here to assist NDIS participants who are eligible or have an approved NDIS plan. Referrals are quick and easy, and can be submitted through our online referral form.

Capacity / Wait List: We aim to see new clients within 1-2 days and can prioritize urgent referrals for quicker access.

Once we receive the referral, we’ll contact you to develop a Service Agreement. This agreement must be approved and signed before services begin. We collaborate with the participant and their decision maker to ensure the agreement meets their needs and provides the best possible support.
